Job Description:
The Department of Community Health and Family Medicine (CHFM) at the University of Florida is seeking a dedicated and skilled Doctor of Pharmacy (PharmD) to join our team. As a Faculty Pharmacist with CHFM, you will shape ambulatory care services, collaborate with primary care clinicians, and drive quality improvement across our clinics. Your efforts will focus on chronic disease management, patient education, and program development that advances our mission of exceptional, community-centered care.
The essential duties of the position are outlined below:
Clinical Patient Care and Consultation:
- Deliver direct patient care through medication therapy management, chronic disease management, and individualized education.
- Collaborate with medical providers to optimize therapy plans, promote medication safety, and support the management of long-term conditions.
- Apply current evidence-based guidelines to therapeutic decisions and document all interventions in the electronic health record.
- Collaborate closely with physicians and other clinicians to develop integrated visit models that enhance access to care.
- Join case reviews, morning huddles, and interdisciplinary rounds, providing timely drug information and consultative expertise.
- Lead and support the development of new pharmacy service lines that enhance clinical outcomes and meet department performance goals.
- Use data to identify gaps, design workflow efficiencies, and monitor indicators such as access, quality measures, and provider productivity.
- Coach patients on their condition, medication use, and lifestyle changes, encouraging shared decision-making and self-management.
- Develop and maintain comprehensive patient education materials and digital tools that enhance adherence and satisfaction.
- Precept pharmacy students and residents, offering guidance, feedback, and career development.
- Collaborate with academic partners to expand experiential learning opportunities.
- Contribute to research, scholarly projects, and continuing education initiatives that advance ambulatory care pharmacy practice.
- Doctor of Pharmacy (PharmD) or equivalent education
- Completion of a PGY1 pharmacy residency
- Completion of required compliance and credentialing requirements per institutional standards
- Demonstrated experience in hospital or ambulatory care clinical practice
- Effective written and verbal communication skills
- Ability to work collaboratively within interdisciplinary teams
- Completion of an American Society of Health-System Pharmacists (ASHP) accredited PGY2 residency with a focus in ambulatory care
- Experience managing chronic disease in outpatient care settings
- Familiarity with informatics or population health tools
